The Care Navigator offers an initial 15 minute complimentary telephone consultation to understand your needs and determine if we can be of assistance in your individual situation.
Consultations - We offer in office and in home consultations. We meet with individuals and families in hospital settings, nursing homes and care communities. Consultations include information gathering, counseling, education, planning and discussion of care coordination. Rate $75-125 per hour
For those interested in information only, we'll advise and help you make decisions to plan. If after the initial meeting you decide you'd like more assistance, we offer an additional option of care coordination.
Assessments - For those interested in an assessment of a current situation, we'll request information, research and make recommendations for your consideration. The rate for the initial consultation applies in addition to an assessment fee of $300.
Care Coordination - We offer a wide range of services - refer to menu of services for additional information. Care coordination is $75 per hour or is quoted on a project basis after we've defined the project together.
Precious Cargo - We've done the research and know how overwhelming the idea of moving can be. We support individuals and families seeking alternative living arrangements like independent living, assisted living etc. This assistance is often called "move management"or housing services. Referral and free services are an industry standard that exist, however we don't believe they work in the best interest of the client. Companies providing this service list themselves as "referral or free services"; it's important to know that referral or free services receive substantial fees for referring or recommending you to retirement and care communities. The question to ask is are they recommending you to the "highest bidder" or the best long term option? If you choose to use this type of service be informed: ask how they're paid and by whom.
We are not a referral or free services. You pay us to represent you. We look at the overall picture of finances, health and personal preferences to determine a long term solution. We provide an assessment, research communities and provide you with options to help you make the best choice for your move. Rate $500-750. Realty, moving services, estate sales and other projects are quoted separately.
